Your before-the-visit checklist
Good preparation helps you put your most important questions into words. Use this checklist for a visit with a licensed physician. It is general educational information, not an invitation to book a visit with Amy Garcia.
- A list of medications and supplements, including doses
- Your health history and relevant records or recent results
- Allergies and any previous medication reactions
- Your top questions, with the most important one first
- Details about any other clinicians involved in your care
Confirm the practical details
Before travelling to an appointment, confirm the office location, date, time and any arrival instructions. Ask what identification or forms are needed and whether records should be sent in advance.
If you use insurance, verify your exact plan, network participation and any referral requirements with both the office and your insurer. A physician’s specialty does not establish insurance coverage.
Make room for your main concern
Write a short note about what has changed, when you noticed it and what you want to understand. You do not need perfect medical language. Bring the questions you would like to ask and a way to record the next steps.
Before you leave
Ask how to receive results, what follow-up is expected and whom to contact with questions. If instructions are unclear, ask the office to explain them again.
For a medical emergency, call 911. This website does not monitor symptoms or provide urgent medical assistance.
